Appointments
USO offices are located in Tulsa and Northeast Oklahoma. The
offices are open Monday through Friday 8am - 5pm, except holidays.
Patients are seen by appointment. Every effort will be made to
schedule a convenient appointment with the physician of your
choice. Please notify our office 24 hours prior to the time of your
appointment to cancel or reschedule. Your time is important to us.
Every effort is made to see patients on time as scheduled.
Occasionally, medical emergencies at our office may require us to
reschedule your appointment.

Insurance
We accept many insurance plans. If your insurance requires
authorization for an office visit, please inform us when you make
an appointment. Also, please let us know if your insurance or
policy has a requirement for "pre-admission certification" or the
need for a "second opinion" prior to a surgical procedure. As a
service to our patients, our office staff will file your primary
insurance claims. If you choose to file your own insurance, a
statement will be provided. If you have any questions or we can
help, please let us know. Duplicate statements are available upon
request. Your medical records are considered personal and
confidential. Before releasing any information to a third party
such as an insurance company, we will require your signature for
authorization.
Payment Policy
Office fees are payable at the time of each office visit unless
prior arrangements have been made. As with any medical care, the
financial responsibility for services provided remains with the
patient or their family. Our business office will be happy to
discuss fees with you. Please feel free to contact the Business

Prescriptions
Routine requests for medicines or prescription refills should be
phoned during weekday office hours. Your medical records will be
available to the doctor to help expedite refill requests.
After-hours prescriptions should be for emergencies only. It is our
policy to not fill narcotic prescriptions after hours. Please
anticipate your prescription refill requirements so we can best
serve your needs.
